Submitting a Prescription—Easy as 1, 2, 3

Although we are unable to solicit prescriptions from your veterinarian on your behalf, we can guide you through the process with these easy-to-follow steps.
Step 1—Veterinarian Writes/Submits the Prescription
Option A—Using a Leedstone Form
Choose one of our prescription forms below.
Prescription Form—Food Animal
Prescription Form—Pet and Companion Animal
Unless specifically refused by you or your veterinarian, Our Leedstone pharmacy has the option to substitute the generic equivalent of prescribed drugs when available.
Option B—Using Your Veterinarian’s Form
The following is required information:
- Veterinarian’s license number
- Veterinarian and/or clinic name, address, phone or fax
- Client’s name
- Product name, strength, size and quantity
- Number of times the prescription can be refilled
- Directions for use; dosage instructions
- Species
- Animal I.D. or group (for livestock or dairy prescriptions)
- Milk and/or meat withholds (for livestock or dairy prescriptions)
- Generics alternative may or may not be used
Your veterinarian can submit your prescription in the following ways. Our Pharmacy hours are: Mon–Fri, 9 a.m.–5:30 p.m. CST; Sat, 9 a.m.–Noon CST. For after-hour emergencies, please contact your veterinarian.

Please Note: We are regulated by state and federal laws and are allowed only to obtain prescriptions from your veterinarian via fax, mail, phone or in person. Your veterinarian can also provide you with the original completed prescription, which you can then submit to us via mail or in person.
